Appointments can be made in person, over the phone or via the internet. Please arrive at least five minutes before your first consultation to allow enough time for registration. You can also download a patient registration form here to fill out and bring in on the day of your appointment.
What to bring
- Referral, x-rays and scans: please bring these to your initial consultation. The more information our practitioners have about your condition the better. These can also be faxed or emailed to our reception desk before your appointment.
- Documentation: if you are making a WorkSafe, TAC, Veterans’ Affairs or ComCare claim please bring the relevant paperwork. It needs to show your claim is current and accepted by the appropriate authority.
- Braces or aids that you are using or have used in the past
- Exercise or gym programs
Referrals are not required except for Veterans’ Affairs or under the Medicare scheme, where a GP referral is necessary. A doctor’s referral is required for WorkSafe claims for exercise physiology and remedial massage.
Medicare and Chronic Disease Management
If you have a chronic problem you may qualify for Medicare benefits under the Chronic Disease Management program. To qualify patients must have a GP Management Plan or Team Care Arrangements prepared by their GP and a current referral from a GP. The program provides approximately $52.95 per session back from Medicare for five visits per calendar year. View our information sheet.
We require a minimum of 24 hours notice if you wish to cancel your appointment. If you do not provide 24 hours notice and you do not attend, we may, at our discretion, charge you for the consultation. TAC and WorkSafe Insurance does not cover charges for non-attendance. These charges will need to be met by the patient. Cancellations need to be made by phone or in person.
Payment is required at the time of consultation. We accept cash, cheque, EFTPOS, Mastercard, Visa and American Express. HICAPS rebates are available to clients with private health insurance. WorkSafe and TAC accounts are the responsibility of the patient until we have confirmation that the claim is accepted.
PhysioSpot, Knoxfield, is located along the service lane on Ferntree Gully Road. We have onsite parking and a disabled car park. There is a ramp for easy access. Both Ventura and Invicta bus lines service the area. The nearest bus stop is less than 100 metres away on Ferntree Gully Road.
Parking at the Wantirna practice is available at the front of the premises or at the Wantirna Mall car park.
We offer a home visit service in the local area for those who are unable to attend the practice. Call us for further information.
